Another Bad Idea: Juvederm on Ebay

In January, we covered the novelty of a woman in Mansfield, Texas selling Botox on Ebay. Her auction and separate website Botox Depot were shut down by Allergan, and the last we heard they were pursuing legal action.


Now there's another batch of Ebay auctions from user JenFangmeier: DIY Juvederm!

She has four different packages of various sizes, and includes syringes of what she claims to be Juvederm with .3% lidocain and needles. On her separate website, DIYJuvederm.com, she is also selling for $90 an instructional CD of her injecting herself with Juvederm.

The savings appear to be significant, as the Buy It Now prices are in line with wholesale prices.

I am unaware of there being a Juvederm with .3% lidocain available in the United States, so who knows what the product actually is.

This is definitely illegal and the very best case scenario is a poor result for someone with Juvederm. Worst case are complications that a DIY injector doesn't know how to deal with.

It is not worth it to take this risk, but some people clearly don't know any better judging from the bids on her auctions.
This is part of a much larger trend... as the economy has declined, incidences of unlicensed cosmetic surgery have jumped. More on that later.

Cosmetic MD Nation Daily Links 3/30

Here's what you need to stay on top of today:

1) Kim Kardashian's Failing Fight Against Cellulite [In Your Face]
From Colin Stewart:
"The last time she shone a spotlight on the subject was last year, when she visited American Laser Centers to try to remove those small lumpy deposits of fat from her thighs. She underwent a Velashape treatment there during an episode of her reality-TV show “Keeping Up with the Kardashians.”This time, her not-perfectly-smooth thighs showed up online. An un-retouched image from a recent shoot for Complex Magazine was inadvertently posted (see below)."

At SMAC Boca, cellulite treatments are performed with an Accent XL, which uses radiofrequency waves to heat the tissue below the skin surface. We also just got Eleme's Smoothshapes, which was launched about a month ago at the AAD meeting in San Francisco. It uses a technology called Photomology, which uses "dynamic laser and light energy with manual manipulation (i.e., mechanical massage) and vacuum" to attack cellulite. Word on the street is it's the best device out there right now.

2. In China, Job Seekers are Resorting to Plastic Surgery [LA Times]
From Don Lee:
"In the U.S., the recession has led to a steep drop in cosmetic surgeries, which generally aren't paid for by health insurers. Nose jobs aren't covered in China either, but that's not stopping consumers here. Job hunters know that a pleasing face helps to get a foot in the door.

"I've been surprised how busy it is," says Dr. Liao Yuhua, president of Shanghai Time Plastic Surgery Hospital, one of the largest in the city. Business began to increase last November, she says, and in recent weeks has been running 40% higher than a year ago. At its busiest in January, Liao says, her team of 10 surgeons was doing as many as 100 procedures a day, raising noses, cutting eyelids and chiseling angular faces into the shape of smooth goose eggs."

Mirroring the trend stories stateside, job-seekers in China are pursuing cosmetic surgery to look their best.

3. New natural boob job uses tummy fat to boost bra size [Mirror.UK]
Emily Cook writes about the advent of stem cell transfers of stomach fat to the breast, to form natural implants. Many centers worldwide are looking at fat grafting to breast for reconstructive and aesthetic use. It's still pretty controversial; it has to do with distinguishing the calcifications from fat grafting from cancers, but strides are being made. Look for this to be mainstream within 10 years.

1. Daily Beauty Reporter: Super Botox Her [Allure Blog]
Allure blogger Erin Flaherty previews the BBC America special Super Botox Me, which will feature British journalist Kate Spicer exploring America's landscape of minimally-invasive cosmetic surgery. She seems to have a much more positive reaction than Morgan Spurlock in Super Size Me, remarking afterward, "I'm sure this isn't the last time I'll be jabbed all over my body in the name of beauty."

2. Rate or Slate Botox for Men? [FemaleFirst UK]
Laura Terry discusses men and cosmetic surgery. Male Botox in Boca Raton and Miami (see Miami Herald piece with Dr. Goldberg) is certainly very popular. In 2008, Botox was up 6% for men according to the latest ASPS statistics.

3. More Hispanics Opt for Nip and Tuck [Washington Times]
Jennifer Harper with an article on America's fastest growing segment of cosmetic surgery patients: 17% more Hispanics had cosmetic surgery in 2008.

1. Downturn Adds a Wrinkle to Cosmetic Procedures [WSJ Blog]
Naureeen S. Malik with a write-up of the 2009 ASPS statistics.
"Women accounted for 91% last year’s cosmetic procedures of all types, with growth seen across age groups above 30 years old. In women ages 20 to 29, surgical procedures fell 9% last year, according to ASPC.

Cosmetic procedures rose 11% among Hispanics, African Americans and Asians to nearly 26% of last year’s total, while procedures among Caucasians fell 2%."


2. The Botox of Tomorrow: Five Cutting-Edge Skin Treatments [Allure]
Lindsy Van Gelder looks into the future of Reloxin, Latisse, Fractional CO2, LED for collagen, and Evolence.
"Evolence, a new injectable, was recently approved by the FDA to plump lips and lines. Some dermatologists believe it will replace injectable collagen, as it doesn't cause allergic reactions."

More plastic surgery statistics: the ASPS releases theirs today

The American Society of Plastic Surgeons, which polls over 6,000 member plastic surgeons but extrapolates to include 24,000 physicians "most likely to perform cosmetic and reconstructive plastic surgery procedures, resulting in the most accurate census available", released their annual statistics today for the year of 2008.

You can find them here.

They paint a much sunnier picture for minimally invasive procedures, which went up 5% compared to 2007. The trend of laser skin resurfacing continues, going up 15%, while Botox boasts an 8% gain next to a 5% gain from soft-tissue fillers.

In this, traditional procedures went down only 9%, with liposuction seeing a big 20% drop. We continue to assert that many of these procedures have shifted over to general practitioners and OBGYNs, to the detriment of patients everywhere.

Where'd the fat go? There's still liposuction in Boca Raton.

The American Society for Plastic Surgery stats from 2008 showed liposuction moved to the #2 surgical procedure from its #1 spot in 2007, being passed by breast augmentation.

The number of procedures performed went from 456k to 341k. Are we taking out less fat? Is this due to economic factors?

I don't think so: the methodology of ASAPS stats is they surveyed over 16,000 board-certified physicians - 6,000 dermatologists, 4,000 plastic surgeons and 6,000 ear/nose/throat docs.

Response rate was a total of 894, and therefore was extrapolated. Since methodology has not really changed from year to year, we can discount the low response rate.

But what about everyone else doing liposuction other than core specialists? I believe that the advent of laser liposuction and the advertising assosciated with it has driven many liposuction procedures to non-core docs - including OBGYN, general practitioners, etc., and that these numbers are not represented in ASAPS statistics.

My belief is the lipo numbers are far greater than in previous years - many are just not in the hands of the docs best trained to do these procedures.

Study: Botox makes you happier?

Sharon Begley at Newsweek reported on a very interesting study published in this month's issue of Cerebral Cortex, by "Andreas Hennenlotter and Bernhard Haslinger of the Tcchnische Universitat Munchen and the Max Planck Institute of Human Cognitive and Brain Sciences", in Germany.
The result?

For Botox patients, however:

"Before Botox, imitating pictures of sad or angry facial expressions caused a large increase in activity in the amygdala, a key emotion region for, especially, anger and anxiety. But after Botox, imitating the angry expressions caused a much lower level of activity in the left amygdala."

Putting on an angry face before you actually feel angry, without Botox, will you angry due to what is called "facial feedback," a concept first outlined by Darwin.

Patients sometimes say they feel happier after Botox, but they just thought it was because they did something nice for themselves...

Can you own the phrase "Locate Plastic Surgeon?"

Interesting, and ridiculous court case filed this week that Real Self.com broke on their blog...

A man named Jamil Ezzo is suing Google, AOL, Time Warner, the American Society of Plastic Surgeons, YellowBookLeads.com, Ienhance.com, Plasticsurgery.com, Findyourplasticsurgeon.com, and DoctorSayYes.com infringing on his trademarked phrase "LOCATE PLASTIC SURGEON."

Although he does own the trademark for that phrase, through some fuzzy math he is seeking damages to the tune of $90,000,000, due to Google and AOL using that phrase in their pay-per-click advertising.

Study continues Reloxin's path to FDA approval: How is it different from Botox?

A study released on Monday afternoon regarding Reloxin is making headlines. Many may wonder why, since it has been discussed in the U.S. media for at least a year and has been used even longer than that in Europe as Dysport.

Dr. Goldberg says:

"This Reloxin study is important because it represents the first published US study involving an alternative to Botox®. It should be noted that although this is the first published US study on Reloxin, this wrinkle softening agent is not new. Reloxin, called Dysport in Europe, is a very popular alternative to Botox® over there. It has been shown to be both safe and effective.

This first US study will be far from the last. We and others are currently involved in several other Reloxin trials. Many of these trials involve evaluating injections of Reloxin every few months and over the course of several years.

Reloxin will be the first US competitor to Botox®. It will not be the last. We and others are studying even more alternatives such as Mentor's Purtox and Merz's Xeomin. The future is exciting for non-invasive cosmetic treatments."

Disclaimer: we know the authors of this study very well and I am on the Reloxin advisory board.

American consumers are interested in the bottom line of this product. Other research seems to say that it has a faster onset than Botox®, and a softer look. It is a good option for using on patients with resistance to Botox®. But generally, Reloxin will be very similar to Botox® and patients should not have any different expectations.

Report: Laser skin resurfacing is recession-proof


The 2008 American Society for Aesthetic Plastic Surgery report polled over 16,000 plastic surgeons, dermatologists and ENT doctors to provide an estimate on the total number of procedures performed last year.

The report drops this morning, and there are three big things you need to know:

1) Almost every procedure is down 10-20%. Total procedures are down 12.3%.
2) Laser skin resurfacing is up 12%, with 570,880 total procedures performed.
3) Breast augmentation has overtaken liposuction as America's most popular surgery.

Patients aged 35-50 continue to dominate, having 45% of the cosmetic procedures, and most commonly getting liposuction and Botox. After the age of 51, the top procedures were blepharoplasty and Botox (32% of the total patient population). For the 19-34 set, the top procedures were breast augmentation and laser hair removal, and for everyone below 18, it was rhinoplasty and laser hair removal.

Dr. Wagner on the Merv Griffin Show

Dr. Kurt Wagner is one of the surgeons on the team at Sanctuary Plastic Surgery in Boca Raton, and made quite a name for himself as a pioneer in plastic surgery.

Here is some groundbreak footage from his appearances on the Merv Griffin Show. Something to note: they discuss the "new trend" of males getting cosmetic surgery... which was also covered in a 1971 Time Magazine article featuring him. At the end of clip five, they also have a great before/after reveal of a great rhinoplasty result that he had with a patient. These are elements of cosmetic surgery coverage that have been used over and over, but they got their start with Dr. Wagner on the Merv Griffin Show.

Introducing Peter Ajello, Candidate for Abdominoplasty

If you haven't heard, I will be donating abdominoplasty and skin tightening surgery to Peter Ajello, a formerly obese man who, so far, has lost 157 pounds on a bet with a friend. His amazing story came to me via The Doctors, the wildly popular CBS medical panel show.

Zeltiq and Photodynamic Therapy star at AAD meeting

Dr. Pozner and I have just returned from the 67th annual American Academy of Dermatology (AAD) meeting in San Francisco and what a meeting it was. Over 18,000 people attended the meeting that dealt with all aspects of dermatology. It should come as no surprise that among the most exciting aspects of the meeting was the ever changing world of cosmetic dermatology.

Fat removal and body contouring are exciting new topics as is the evolving world of photodynamic therapy. Cryolipolysis by Zeltiq continues to show enormous interest. This totally non-invasive method of melting fat is currently undergoing FDA trials, but has just received regulatory approval in Europe, as Dr. Pozner reported below this post. Presentations showed remarkable melting of love handles simply by selectively applying “cold” to the areas of fat. This approach added to other future methods of non-invasive body contouring by UltraShape and Liposonix (now owned by Medicis the company that sells the Restylane wrinkle filer) only add to the excitement.

Photodynamic therapy (PDT) also was another exciting area of discussion. This approach of applying a medication to the skin that is then activated by a laser or light source has been used for years to treat pre-skin cancers. More recently the technique has been shown to promote better skin quality and lead to improvement in acne. What is most exciting was the suggestion that newer PDT approaches currently being studied may ultimately lead to a cure for acne and better ways to remove unwanted hair. Stay tuned!

Fat Wars: Zeltiq update from American Academy of Dermatology Meetings


Update on the fat wars - Zeltiq (www.zeltiq.com) - has a unique system for fat removal through cryolipolysis that we've blogged about a few times.

I just learned that it received its CE mark, which is the European equivalent to FDA approval. They are now able to sell the device in Europe. The FDA's studies are ongoing, however.

Guest Blogger: Dr. Julius Few, Chicago

Here I am with Dr. Julius Few at the Brazilian-American meeting in Utah. Yes, we actually went to the meeting. His guest blog follows after the picture.

It was an honor to attend the first American-Brazilian Meeting in Utah. The long weekend was literally perfect, but the content of the meeting was even better. Despite the temptation for participants to go out to nature during the meeting, most waited until the allotted break time because the content was so dynamic.

This meeting represented a unique mix of aesthetic medicine and surgery. I found the focus on how to keep our practices up to date incredibly useful. I believe the focus on non-surgical, laser, and cosmetic medicine very beneficial. Our patients are coming into our offices wanting the state of the art, with the least amount of down time and the most “bang for the buck”. I believe the emphasis on what works in lasers and what doesn’t was very useful-Dr. Pozner and Dr. Dibernardo gave very informative presentations.

I had the honor of participating in a panel focused on aesthetic medicine and the wave of the future. I feel that patients expect a comprehensive approach to their rejuvenation experience and the ability to offer surgical and non surgical options are vital. On a side note, I found the input of our international counterparts very innovative and “out of the box”. I really liked the minimally invasive way to lyse/cut platysmal bands very useful and I give our Brazilian friends a great deal of credit. I can’t believe I did not think of using a suture placed through the skin to cut muscle bands without a scar, an inexpensive surgical wire!

Dr. Pozner to be on The Doctors

It was announced today on CBS' The Doctors that I will be performing a tummy tuck (aka abdominoplasty) on Pete, a 35-year old man who went from 405 pounds to 248 in six months.


From the show synopsis:
"Weighing 405 pounds and wearing size 58 pants just seven months ago, Pete lost 50 pounds by just changing his diet and exercising more often... But his friends weren't satisfied and offered Pete $15,000 to lose 100 additional pounds in six months. By sticking to a rigorous workout regimen and a healthy diet that included berries, egg whites, oatmeal and sweet potatoes, the pounds melted away for Pete so quickly that his friends tried to sabotage him by sending him pizzas and putting donuts in his gym bag! Pete fought off the temptations and kept losing the weight. "

He has to lose another 50-60 pounds in order for the surgery to make sense, and we'll keep you posted on how it goes and when he will return to the Doctors. Thank you to everyone who called the office after hearing about us today!

Guest Blog: Dr. Diane Duncan on RFAL assisted liposuction

I just returned from the inaugural Brazilian-American meeting of plastic surgeons held last week by the ASAPS and ISAPS in beautiful Park City, Utah. The meeting was fantastic and melded some terrific techniques and presentation from both Brazilian and American physicians. This meeting was the brainchild of Renato Saltz, MD, an internationally respected plastic surgeon in Salt Lake City by way of Brazil.



One of the best presentations was by Diane Duncan, MD(pictured above), a leading plastic surgeon from Colorado who presented her data on Invasix, with whom she's been doing clinical trials for their RF liposuction/skin retraction device. On their website, it is billed as the first "RFAL," or Radio Frequency Assisted Liposuction device. What follows is a guest post by Dr. Duncan:

"One of the most exciting new technologies we have come across is made by a new Israeli company Invasix. Their device is a radiofrequency modulated liposuction and skin retraction device, currently being reviewed by the FDA for approval.

I have been doing some phase III clinical trials. This device is used internally during liposuction and caused unidirectional heating of the skin. The heating is more uniform than can be achieved with laser liposuction and quicker. The device also can control the amount of skin heating by decreasing energy when the skin achieves the correct temperature. This machine seems to offer a real advantage in obtaining localized skin surface area reduction without skin excision.

Many patients that we would have previously treated with an abdominoplasty may achieve excellent results with radiofrequency assisted liposuction alone. The next study will be a split abdomen study, comparing traditional liposuction to the radiofrequency modality. A 3-D CAD system will be used to measure both volumetric and skin surface area changes."

The importance of LED treatments

Light Emitting Diode (LED) technology is becoming an increasingly more important aspect of many medical and cosmetic treatments. In fact the varied benefits of LED treatments are very analogous to the multiple benefits of Botox® injections. Botox® injections can lead to profound benefits both in neurology and cosmetic medicine. Botox®, as everybody knows, is wonderful in its ability to decrease wrinkles and excess underarm sweating. In addition, Botox® can also markedly improve migraine headaches or the stiffness associated with cerebral palsy or after certain strokes. LED treatments also have found their way into many medical fields

LED treatments are in some ways like laser treatments and in some ways are very different. Just like different lasers emit different colors of light and thus different lasers are used for wrinkles, age spots, acne or spider veins, LED light technology also comes in many different colors. There are LED machines that emit, blue, red, yellow and near-infra-red light. Each different LED has a different effect on different cells and therefore each has potential different uses.

What makes LEDs so very different than lasers is their effect on the skin – they are without any of the heat effect seen with most lasers. Because there is no heat produced from LED machines, treatments are painless and safe for all skin types. In research done in our offices we have shown that blue and red LED treatments can be very effective in treating acne, while red and near-infrared LED treatments are helping in promoting anti-aging of the skin. These studies have now been published in well respected journals.

In addition to the cosmetic skin benefits of LED treatments, recent studies have shown that yellow LED treatment can lessen the severity of a sunburn and now red LED treatment has been shown to decreased the discomfort of radiation therapy following the treatment of breast cancer.

LED treatment currently is, and will continue to be, an important adjunct therapy to many aspects of cosmetic and medical treatment.
